Extended Aged Care at Home Dementia (EACHD) are individually planned and coordinated packages of care tailored to help frail older people who have been assessed as experiencing behaviours of concern and psychological symptoms associated with dementia.

In addition to the services provided under CACP and EACH, EACHD facilitates access to behavioural management specialists and the use of strategies and service approaches to address the underlying causes of behaviours of concern.

The Forrest Centre is funded to provide care for clients in Tumut.

The service is funded by the Commonwealth Department of Health and Ageing.
