ComPacks is a program for case-managed discharge from Wagga Wagga, Griffith, Narrandera and Leeton hospitals. The eligibility requirement is a need for case management and two or more community services to enable people of any age to return home safely with appropriate care for up to six weeks.

Community support is jointly case managed with a multidisciplinary hospital team, before and after discharge, with the aim of facilitating access to mainstream community services for up to six weeks, and if they are required, beyond that period.

The service involves the community assessment and case management of targeted people with the purpose of rapidly assembling individualized community care packages. It’s designed to meet each person’s assessed clinical and support needs.

Services may include:

• Domestic assistance
• Personal care
• Meals
• Social support
• Transport
• Respite care
• Centre based day care

Referrals are made through hospital staff such as discharge planners and social workers.

There is a fee for the six week participation which includes case management and brokered services. Usual fees apply to community services.
