Community Options provides case management services for people with disabilities, frail older people with complex care needs and their carers.

This Home and Community Care (HACC) program supports clients in Wagga Wagga, Griffith, Leeton, Narrandera, Carrathool and Murrumbidgee local government areas.

Community Options coordinates care requirements through existing Home and Community Care providers with brokerage capacity to purchase additional services unavailable through the community care network.

Services may include:

• nursing care
• personal care
• assistance with shopping
• light housework
• meal preparation
• laundry

The program is jointly funded by the Commonwealth and State Governments and is administered through the NSW Department of Human Services: Ageing Disability & Home Care.
