CACP provides coordinated community care for frail older people who have complex care needs and who are eligible for low level residential care.

The Aged Care Assessment Team (ACAT) assesses clients to determine eligibility for access into the program.

The Forrest Centre is funded to provide care to clients in Wagga Wagga, Tumut and Tumbarumba,

To support people living in their own homes, services can include help with:

• bathing
• dressing
• meal preparation
• housework
• laundry
• transport
• temporary in-home respite
• home maintenance; and
• social activities.

The services are delivered in a flexible way to meet the changing needs of clients.

The service is funded by the Commonwealth Department of Health and Ageing.
